Whistle-Blowing Form

Please fill the following details and submit. QNCC encourages the whistle blower to disclose his/her identity when raising whistle-blowing allegations. QNCC will do its best to protect the identity of the whistle blower. Concerns expressed anonymously are much less powerful but are nevertheless considered at the discretion of Audit Committee. In exercising the discretion, the factors to be taken into account would include:

1. The seriousness of the issue raised.
2. The credibility of the concern.
3. The likelihood of confirming the allegation from other sources.

Note:

Untrue Allegations: If a whistle-blower makes an allegation in good faith, which is not confirmed/proven by the ensuring investigation, no action will be taken. If, however, the whistle-blower makes malicious or vexatious allegations, appropriate disciplinary action will be initiated against the concerned person.
